Transvaginal ultrasound vs Pelvic exam?
Hi, I am 33 years ago and been suffering from brown spotting for over a month. I went to my gyn today and he did the pelvic exam. He said that everything appears to be normal and wanted to give me hormone pills. I'm in the process of getting pregnant so I really want to stay away from bc bills. However, I am dying to know what's wrong with me. I already had another dr performed the pelvic exam and blood test and also pelvic ultrasound. They all came back normal. However, I want a second opinion so i went to him and I request transvaginal ultrasound because i was told it gives you more detail. But then he told me that it's useless because he already checked my pelvic and it seems ok. He can't feel any cysts or fibroids He told me that he can order biopsy for me to see if there's any abnormal cells. My question is can cysts and fibroids or even polyps detect from pelvic exam? I heard lots of people found out from ultrasound but not pelvic exam. Please help
Yes, sometimes cysts and fibroids can be felt on a pelvic exam but not always.
I had a cyst in 2002/2003 and my doctor felt a mass after I told her I felt something. Well, when I went to have an ultrasound done it didn't show anything. It wasn't til I had surgery that then we knew what the mass was: a functional cyst.
I've also had a polyp before, in 1999 and my doctor found that via a pelvic exam as well. I don't know how big it was but I had it removed right there in the office.
I would recommend getting an additional opinion preferably from someone not in the office of the doctor you just saw, also talk to your primary care physician to see if they will send you for the transvaginal ultrasound.
The two ways to examine often work together to try and find out what's going on. Also, with an ultrasound they can check the lining of the uterus to see if it's too thick or too thin.
Also, ask your doctor if he took cultures to check for infection. Sometimes infections can cause bleeding as well. Infections such as cervical infections can also cause bleeding like what you are describing.
Sometimes it takes a 3rd or 4th opinion to try and find out what's wrong.

Women Infertility Surgery
Surgical procedures for women include diagnostic hysteroscopy, diagnostic laparoscopy, tubal reversal, assisted reproductive technology procedures (GIFT and ZIFT), and reconstructive pelvic surgery. Highly trained and experienced doctors can often use minimally-invasive surgery instead of abdominal surgery to minimize pain, scarring, and recuperation time.
Procedures for men, usually performed by a urologist, include vasectomy reversal, removal of obstructions, sperm aspiration, and varicocele repair.
Procedures for women
Several surgical procedures can help you improve your chances for pregnancy. Abdominal surgery was the traditional way to gain access to the internal pelvic organs. Now, most procedures can be done using laparoscopy or hysteroscopy. These less-invasive procedures minimize scarring and recovery time as well as complications.
Diagnostic hysteroscopy
Your doctor may use a hysteroscopy for diagnosis if your ultrasound exam suggests problems inside the uterus or gives incomplete information. A hysteroscopy allows your doctor to examine the uterus for scar tissue, polyps, or fibroids. A telescope-like device is passed through the cervix. Hysteroscopy requires no incision, and patients usually recover quickly.
If any problems are found, your doctor may be able to correct them during the initial hysteroscopy. Follow-up may be necessary with a separate operative hysteroscopy or other surgical procedure.
